We are entering a time that may well offer the research breakthroughs and the answers we need for a cure. That is what FSR is committed to doing for patients, their families and their children - improving care for sarcoidosis patients and finding a cure for this disease. We hope you will consider making a contribution to support FSR's Annual Fund. Your gift of $25, $50, $100 or more will support FSR’s independent research programs and other crucial initiatives to educate health professionals and the public about this little known, but potentially fatal disease.  Donations to FSR are tax-deductible.  Please consider making a donation today!

There are three easy ways to make a donation to FSR:

  • Support the 2008 KISS Campaign:  There are numerous events in February that provide an opportunity to support the 2008 Campaign.

  • Online: Use our Web site's secure server to make a general donation, or a donation "in honor of" or "in memory of" a friend or loved one.  Feel confident that your credit card transaction is safe and confidential

  • By Telephone: Please call 312-341-0500 and our staff will be pleased to take your information.

  • By Mail: To make a donation by mail, please make your check payable to the Foundation for Sarcoidosis Research and mail it to:

    Foundation for Sarcoidosis Research
122 South Michigan Avenue, Suite 1700
    Chicago, IL 60603

Other ways to contribute:

Matching Gifts/Workplace Giving
Many companies will match their employees contributions to qualifying nonprofit organizations, often doubling or sometimes even tripling the amount of the donation. Your company may have a matching gift program even though you have never heard about it. Contact your Human Resources Department for a copy of their matching gift form. Complete this form and include it with your donation. FSR will fill in the required verification information and return the form to your company. If you have any questions, please call us at 312-341-0500 or e-mail us at info@stopsarcoidosis.org.

United Way
If you contribute to a local United Way campaign through a payroll deduction, your local United Way Chapter may allow you to direct a portion of your contribution to the Foundation for Sarcoidosis Research.  To do so on your United Way pledge form, please indicate that you want to support either the "Donor Choice Program" or "Other 501c3 Organization" and then add the following information:

Foundation for Sarcoidosis Research
122 South Michigan Avenue, Suite 1700
Chicago, IL 60603
Federal ID#: 36-4378232

Local Fundraising Events
Are you planning a birthday party, holiday party or anniversary? Why not make FSR the beneficiary of your next celebration? Instead of gifts, ask your guests to make a charitable contribution to FSR.  Donations are tax deductible, and all donors will receive an acknowledgment letter for tax purposes.

You can also help FSR raise awareness of this disease and funds to support much-needed research throughout the year by hosting a K.I.S.S. (Kick In to Stop Sarcoidosis™) event in your area.  Learn more.

Wells Fargo Sharing Advantage
Through an exciting program called 'Sharing Advantage', when you purchase a new home using Wells Fargo Private Mortgage Banking, or when you refinance your existing non-Wells Fargo mortgage through Wells Fargo Private Mortgage Banking, Wells Fargo will make a donation of $300.00 in your name to FSR.

Properties may be located anywhere in the United States.

When you are ready to purchase or refinance your personal residence, your vacation home or investment properties, up to 4 units, please contact: Frantz Osse, Wells Fargo Private Mortgage Banking, at 888-333-6584. You may also reach him via e-mail at ossefr@wellsfargo.com.

GoodSearch.Com
Raise money for FSR whenever you search the Internet with GoodSearch powered by Yahoo!  It's a search engine just like you're used to (it's powered by Yahoo), but the first time you go on the site, you can designate FSR as your favorite charity. Then every time you search, money will go to the Foundation.

Visit GoodSearch, follow the instructions to select the 'Foundation for Sarcoidosis Research (FSR)' as your charity (search 'sarcoidosis' to find us), then click 'Verify'.  Every time you use this search, money is generated to support FSR.

With each search you launch on GoodSearch, about 1 cent goes to FSR. The more people who use this site, the more money will go support the important work of the Foundation.  For example, if 1,000 people search FSR twice a day for a year, we earn $7,300.  So please spread the word to your friends and family.

Planned Giving
Whether your estate is large or small, you can support FSR beyond your lifetime by including a gift to FSR in your will. To make a bequest of cash, securities, or other property to the Foundation for Sarcoidosis Research, your will should state:

"I give and bequeath to the Foundation for Sarcoidosis Research, a non-profit
organization with Federal ID #36-4378232, having its principal office at
122 South Michigan Avenue, Suite 1700, Chicago, IL 60603..." and then indicate a dollar amount, a percentage of your estate or a specific asset (for example, stock, securities).

If you would like more information on making a bequest of stock, securities, life insurance or including FSR in your will, please call us at 312-341-0500 or e-mail us at info@stopsarcoidosis.org.

Research Fund
The FSR Board of Directors feels strongly that the causes of this disease, and ultimately a cure, will be uncovered through rigorous and extensive scientific research.  The FSR Scientific Advisory Board, which includes many preeminent sarcoidosis researchers and clinicians, oversees the Foundation’s independent research program.  FSR would like to be able to double the amount that we spend on research, but need your help to do so.

FSR appreciates 'unrestricted' gifts because we can use the funds to support programs considered most urgent at the time we receive your donation. However, you can specify how we use your gift. For example, a restricted purpose would include a donation for research.  If you plan to make a restricted gift for research, please contact us to make certain we can meet the conditions you specify.
